Complete it and mail it along with your payment (checks made out to University of Kansas) to Laura Razo at her address listed on the registration form.

Here is a breakdown of the cost for planning purposes:

Early registration (till July 1, 2002): GNSI $100.00; non-GNSI $140.00
Day Registration: Each day GNSI $40.00; non-GNSI $55.00
Annual Banquet $30.

(Entrees will be as listed; the chef plans a new menu for each month and might prepare the entrees with different seasonings and sides. He uses local, fresh ingredients, so availability helps determine menu)

Registration policy:
All forms must include payment and be postmarked by July 1, 2002 to qualify for early registration fee (after this date, please include the $50.00 additional registration fee). Registrations canceled before July 22 are eligible for a 50% refund.
