Conference Registration Form

Please add the following charge to the Registration Form.

1. The information for international memberships is incorrect.
   Internationall membership should be $55+$20 for airmail, for a total of $75 -- NOT $95. Members do not need to add $20 for airmail to total $95.

The $5 credit card transaction fee mentioned earlier is no longer in
effect. You do not need to add $5 to your credit card registration
total.
